拉萨市中小学生包虫病健康教育干预效果评价

    Evaluation of the effectiveness of health education interventions on echinococcosis among primary and secondary school students in Lhasa city, China

      目的 分析评价拉萨市中小学生包虫病防治健康教育干预效果, 为探索藏区学生包虫病防治健康教育模式提供依据。
      方法 随机抽取拉萨市城镇(城关区)、纯农业区(曲水县)、纯牧业区(当雄县)和半农半牧区(林周县)小学、初中、高中共12所学校约1 440名学生, 从2020—2022年通过知识讲座的方式连续3年进行包虫病健康教育。在干预开始前和结束后分别对中小学生包虫病防治知识和行为进行问卷调查, 采用包虫病核心知识知晓率、行为以及态度正确率的前后比较来评价干预效果。
      结果 在进行健康教育干预3年时间里, 拉萨市中小学生的平均包虫病核心知识知晓率逐渐增加。与干预前(44.6%)相比, 核心知识知晓率在干预的第2年(71.6%)和第3年(99.4%)差异均有统计学意义(均P < 0.001)。女生(知晓率分别为55.4%、73.0%、99.0%)、高中生(60.9%、78.9%、98.3%)、城关区(53.3%、74.2%、98.9%)学生的知晓率在干预的第1年到第3年显著增加, 与干预前(42.4%、45.9%、42.7%)相比差异均有统计学意义(均P < 0.001)。学生的包虫病防治行为不接触狗(84.8%)、坚持洗手(88.4%)、从不喂狗吃生的家畜脏器(93.0%)及态度如果有生的家畜病变脏器不会直接喂狗(98.8%)的平均正确率在干预的第3年显著增加, 与干预前(分别为36.8%、55.7%、59.2%、86.4%) 相比差异均有统计学意义(均P < 0.001)。在干预的第3年纯牧区(当雄县)和半农半牧区(林周县)学生核心知识知晓率和行为3的正确率均达到了100%。
      结论 对拉萨市中小学生连续进行包虫病健康教育干预的效果优于单次干预, 纯牧区和半农半牧区学生的干预效果显著。

       

      Abstract:
      Objective To analyze and evaluate the effectiveness of health education interventions for echinococcosis prevention and control among primary and secondary school students in Lhasa, and to provide a basis for exploring health education models for the prevention and control of echinococcosis among students in Tibetan regions.
      Methods A total of 1 440 students were randomly selected from 12 primary, junior high, and senior high schools in urban (Chengguan district), purely agricultural (Qushui county), purely pastoral (Dangxiong county), and semi-agricultural semi-pastoral (Linzhou county) regions of Lhasa. From 2020 to 2022, a three-year health education program on echinococcosis was conducted through structured knowledge lectures. Questionnaires on echinococcosis prevention knowledge and behaviors were administered before and after the intervention. The effectiveness of the intervention was evaluated by comparing pre-and post-intervention rates of core echinococcosis knowledge awareness, correct behaviors, and correct attitudes.
      Results Over the three-year health education intervention, the mean awareness rate of core knowledge about echinococcosis among primary and secondary school students in Lhasa gradually increased. Compared with the pre-intervention awareness rate (44.6%), the awareness rates in the second (71.6%) and third (99.4%) years of the intervention showed statistically significant differences (both P < 0.001). Female students (55.4%, 73.0%, and 99.0%), high school students (60.9%, 78.9%, and 98.3%), and students from Chengguan district (53.3%, 74.2%, and 98.9%) exhibited significant increases in awareness rates from the first to the third intervention years, with statistically significant differences compared to pre-intervention awareness rates (42.4%, 45.9%, and 42.7%) (all P < 0.001). The mean correct rates of the students for echinococcosis prevention and control behaviors, "avoid contact with dogs"(84.8%), "maintain handwashing habits" (88.4%), "never feed dogs with raw livestock viscera" (93.0%), and attitude "won't feed dogs with raw diseased livestock viscera directly" (98.8%), showed significant improvements by the third year of the intervention, with statistically significant differences compared to pre-intervention rates (36.8%, 55.7%, 59.2%, and 86.4%) (all P < 0.001). In the third year of the intervention, the core knowledge awareness rate and the correct rate for Behavior 3 among students reached 100% in both the purely pastoral (Dangxiong county) and semi-agricultural semi-pastoral (Linzhou county) regions.
      Conclusion Continuous health education interventions on echinococcosis prevention demonstrated superior effectiveness compared to one-time interventions among primary and secondary school students in Lhasa, with particularly significant improvements observed among students from purely pastoral and semi-agricultural semi-pastoral regions.
