极端气温干预措施健康有效性评价综述

    Review on the health effectiveness evaluation of extreme temperature intervention measures

      目的 系统综述极端气温干预措施对人群健康的影响。
      方法 检索和分析中国知网、万方、PubMed及Web of Science数据库相关文献及其参考文献。
      结果 最终纳入58篇文献, 27篇为时间序列研究, 31篇为实验性研究, 涉及多种干预措施和多个健康结局。在高温环境下, 国家预警与行动措施能降低总死亡率、极端温度相关的患病率和就诊率。同时, 防护装备和环境温度调控干预可改善体温调节、汗液分泌、心血管功能、缓解热应激症状和提升主观舒适感。健康教育行为指导可有效提升公众防护认知水平与行为依从性, 改善睡眠及心理状态, 减少热相关疾病发生。而低温环境下, 寒冷预警、房屋改造和供暖干预可降低全因死亡率及心肺疾病风险, 但相关证据尚不充分。
      结论 极端气温的干预措施对健康改善有积极作用, 脆弱人群受益明显, 但低温干预相关研究证据缺乏, 未来需开展长期随访完善相关证据。

       

      Abstract:
      Objective To systematically review the impact of extreme temperature intervention measures on population health.
      Methods A retrieval and analysis was conducted in databases including Chinese National Knowledge Infrastructure (CNKI), Wanfang, PubMed, and Web of Science, and references of the retrieved literature were traced.
      Results A total of 58 articles were included, of which 27 were time-series studies and 31 were experimental studies, covering various intervention measures and multiple health outcomes. In high-temperature environments, national early warning and response measures reduced overall mortality, extreme temperature-related morbidity, and medical consultation rates. Meanwhile, protective equipment and environmental temperature control interventions improved thermoregulation, sweat secretion, and cardiovascular function, thereby alleviating heat stress symptoms and enhancing subjective comfort. Health education and behavioral guidance effectively increased public awareness of protection and behavioral compliance, reduced the incidence of heat-related illnesses, and improved sleep and psychological well-being. In low-temperature environments, cold weather alerts, housing retrofits, and heating interventions might lower all-cause mortality and cardiopulmonary disease risks, though relevant evidence remained insufficient.
      Conclusion Extreme temperature intervention measures have a positive impact on health improvement, with vulnerable populations benefiting more significantly. However, evidence regarding low-temperature interventions is still insufficient. Future studies should include long-term follow-ups to strengthen the evidence base.
