    基于结构方程模型的武汉地铁职工血脂异常影响因素研究

    Study on influencing factors of dyslipidemia in Wuhan subway staff based on structural equation model

      目的 了解武汉市地铁职工血脂异常检出情况,利用结构方程模型探讨其血脂异常影响因素的直接和间接作用,为职工健康风险管理提供依据。
      方法 研究基于武汉地铁集团与武汉市疾病预防控制中心合作开展的地铁职工健康调查项目,选取2018年5月-2019年10月项目调查中完成问卷调查,并同时完成健康体检的6 385名地铁职工作为研究对象。采用SAS 9.4软件对数据进行统计分析,采用Amos 21.0软件构建血脂异常患病影响因素的结构方程模型。
      结果 武汉市地铁职工血脂异常患病率为15.00%,以高甘油三酯血症(66.70%)、高总胆固醇血症(13.26%)为主。结构方程模型显示,年龄对血脂异常患病有直接和间接作用,其总路径系数为0.17。生理状况对血脂异常患病的直接路径系数为0.52,其中以高血压对血脂异常患病的直接影响最大,其路径系数为0.24;现在吸烟对血脂异常患病有直接作用,路径系数为0.07,表明现在吸烟者血脂异常患病风险比不吸烟者高0.07(P值均 < 0.01)。
      结论 武汉市地铁职工血脂异常的患病率处于较低水平,但仍应加强健康教育提高职工对血脂异常危害的认识,重视血脂、血压等指标的监测,以降低血脂异常的发生风险。

       

      Abstract:
      Objective To investigate the prevalence of dyslipidemia among subway staff in Wuhan, China, and explore the direct and indirect effects of factors affecting dyslipidemia by using structural equation modeling, so as to provide evidence for health risk management the risk of health for staff.
      Methods Based on the subway employee health survey project conducted in collaboration between Wuhan Metro Group and Wuhan Center for Disease Control and Prevention, a total of 6 385 subway staff who completed the questionnaire survey and physical examination were selected as the research objects from May 2018 to October 2019. Data were analyzed by SAS 9.4 software, and the structural equation model of influencing factors of dyslipidemia was constructed by Amos 21.0.
      Results The prevalence rate of dyslipidemia among Wuhan subway staff was 15.00%, mainly hypertriglyceridemia (66.70%) and hypercholesterolemia (13.26%). The result of structural equation model showed that age had direct and indirect effects on the incidence of dyslipidemia, with a total path coefficient of 0.17. The direct path coefficient of physiological conditions for the prevalence of dyslipidemia was 0.52, in which hypertension contributed most with a path coefficient of 0.24. Current smoking showed a direct effect on the prevalence of dyslipidemia, and the path coefficient was 0.07, indicating that risk of dyslipidemia in current smoking was 0.07 higher than that in non-smoking (all P < 0.01).
      Conclusion The prevalence of dyslipidemia among Wuhan subway staff was at a low level, but it was still necessary to strengthen health education to improve staff's awareness of the harm of dyslipidemia, and pay more attention to monitoring of blood lipids, blood pressure, and other indicators so as to reduce the risk of dyslipidemia.
