    2021年唐山市某医院环境微生物监测结果分析

    Environmental microorganisms monitored in a hospital in Tangshan, China, 2021

      目的 了解唐山市某医院环境微生物监测情况及重点区域微生物学分布特征,以便加强医院环境卫生管理。
      方法 对2021年1—12月在唐山市某医院的7个重点区域采集空气样本、物体表面和医护人员卫生手样本,按照《全国临床检验操作规程》(第4版)操作进行细菌培养和菌株鉴定,测定样本中的菌落总数,每月监测1次。
      结果 2021年唐山市某医院重点区域样本达标率为96.8%,其中室内空气、物体表面、医护人员卫生手样本达标率分别为为97.1%、95.7%、96.9%。空气、物体表面、医护人员卫生手样本菌落总数分别为(2.4±0.3)CFU/皿、(3.2±0.3)CFU/cm2、(3.1±0.2)CFU/cm2。不同重点区域样本菌落总数总体达标率差异有统计学意义(χ2=40.65,P < 0.05),急诊、CT室分别与门诊、发热门诊总体达标率差异有统计学意义(P < 0.002 27)。不同重点区域空气样本菌落总数达标率差异有统计学意义(χ2=44.23,P < 0.05),急诊、CT室空气样本与门诊达标率差异有统计学意义(P < 0.002 27);不同重点区域物体表面、医护人员卫生手样本菌落总数达标率差异无统计学意义(P>0.05)。对140个不达标样本共培养及鉴定出5类14种165个菌株,以凝固酶阴性葡萄球菌、人葡萄球菌、枯草芽胞杆菌为主,占比分别为35.7%(59/165)、23.6%(39/165)、23.6%(39/165)。
      结论 该医院重点区域菌落总数达标率总体较好,微生物分布以革兰氏阳性球菌为主,发生医院感染风险较低,但急诊科及CT室仍需进一步加强日常消毒及微生物监测。

       

      Abstract:
      Objective To investigate the environmental microbial monitoring and its distribution characteristics in key areas of a hospital in Tangshan, China, so as to strengthen the management of hospital environmental sanitation.
      Methods Air samples, object surface samples, and hygienic hand samples of medical staff were collected from seven key areas of a hospital in Tangshan during the 12 months of 2021. Bacterial culture and strain identification were performed according to the 4th edition of the National Clinical Laboratory Practice. The total number of colonies in the samples was determined once a month.
      Results In 2021, the qualified rate of samples from key areas of a hospital in Tangshan was 96.8% (97.1% of indoor air samples, 95.7% of object surface samples, and 96.9% of hand hygienic samples). The total numbers of colonies in air, object surface, and hygienic hand samples were (2.4±0.3) CFU/plate, (3.2±0.3) CFU/cm2, and (3.1±0.2) CFU/cm2, respectively. There were significant differences in qualified rate of the total number of colonies among different key regions (χ2=40.65, P < 0.05). Further comparison between two groups showed significant differences between emergency department and outpatient department as well as between CT room and fever outpatient department (P < 0.002 27). There was a significant difference in the total number of colonies in the air samples from different key areas(χ2= 44.23, P < 0.05). Outpatient department showed significant differences in qualified rate as compared with emergency department and CT room (P < 0.002 27). There were no significant differences in the total number of colonies in the object surfaces samples and hygienic hand samples among key regions (P>0.05). A total of 165 strains of 14 species in 5 genera were identified from 140 unqualified samples, mainly including coagulase negative staphylococcus, Staphylococcus hominis, and Bacillus subtilis, accounting for 35.7% (59/165), 23.6% (39/165), and 23.6% (39/165), respectively.
      Conclusion The qualified rate of the total number of colonies in key areas of the hospital is generally high, and the identified microbes are mainly gram-positive cocci. The risk of hospital infection is low, but the emergency department and CT room still need to further strengthen daily disinfection and microbial monitoring.
