    寒潮与燃煤取暖相关一氧化碳中毒风险的关联性研究——以济南市为例

    Association between cold wave and carbon monoxide poisoning risk caused by household coal heating: a case study of Jinan

      目的 以济南市为例,分析家庭燃煤取暖造成的一氧化碳(carbon monoxide,CO)中毒事件的时间分布,探讨寒潮与CO中毒急性效应的关联。
      方法 收集济南市2007—2021年家庭燃煤取暖造成的CO中毒事件,采用时间分层病例交叉设计,评估了10种不同寒潮定义及其滞后0~10天对CO中毒的影响。
      结果 2007—2021年济南市共有4 093例家庭燃煤取暖造成的CO中毒事件,研究发现,寒潮与济南市CO中毒风险的增加有关。分别以P01P05P10作为寒潮温度阈值时,最大效应水平分别出现在定义P01(3)、P05(5)和P10(5)滞后6天时,其比值比分别为2.58(95%CI: 1.59~ 4.19),2.06(95%CI: 1.57~2.7)和1.62(95%CI: 1.36,1.93)。
      结论 不同寒潮定义与CO中毒的关联存在差异,但随着低温阈值的降低和寒潮持续时间的增加,寒潮对CO中毒的关联也相应增加,寒潮与CO中毒风险之间存在显著相关性。

       

      Abstract:
      Objective In case of Jinan to analyze the time distribution of carbon monoxide (CO) poisoning events caused by household coal heating in Jinan, and to explore the acute effect of cold wave on CO poisoning.
      Methods The CO poisoning events caused by household coal heating in Jinan from 2007 to 2021 were collected. A time-stratified case-crossover was performed to evaluate the impact of 10 different cold wave definitions and its lag of 0-10 days on CO poisoning.
      Results From 2007 to 2021, there were 4 093 cases of CO poisoning caused by household coal heating in Jinan. The cold wave was associated with the increased risk of CO poisoning in Jinan. When P01, P05 and P10were used as temperature thresholds for cold wave, the most significant effects were observed when P01 (3), P05 (5) and P10 (5) lagged for 6 days, with and OR of 2.58 (95%CI: 1.59-4.19), 2.06 (95%CI: 1.57-2.7) and 1.62 (95%CI: 1.36-1.93), respectively.
      Conclusion There were different effects on CO poisoning for different temperature thresholds of cold waves; while, with the decrease of low temperature threshold and the increase of cold wave duration, the impact of cold waves on CO poisoning was increasing accordingly, and there was a significant correlationship between cold wave and risk of CO poisoning.
