    围孕期母亲空气污染暴露与新生儿出生体重相关性分析

    Periconceptional exposure to air pollution with birth weight of neonates

      目的 探讨围孕期母亲空气污染暴露与新生儿出生体重的相关性。
      方法 2017年1月至2020年1月对南宁市某三甲医院建档的孕妇进行3年的随访调查, 采用面对面问卷调查的方法收集孕妇一般资料。收集受试者孕前期(孕前12周)及孕早期(末期月经至孕12周)南宁市环境空气质量指数(Air Quality Index, AQI)、PM2.5、PM10、SO2、NO2及CO浓度监测数据。采用Logistic回归模型分析母亲围孕期空气污染暴露对新生儿低出生体重的影响。
      结果 本研究共纳入调查的1326例孕妇均顺利分娩, 低出生体重儿共42例(3.17%)。南宁市冬季环境空气AQI指数、PM2.5、PM10、SO2、NO2及CO水平均高于其他季节。多因素分析结果显示: 孕前期及孕早期AQI升高、妊娠期高血压、前置胎盘、胎盘早剥为危险性因素, OR值(95%CI)分别为1.619(1.114~2.355)、1.531(1.087~2.158)、1.603(1.151~2.233)、1.519(1.121~2.058)和1.478(1.045~2.092)。
      结论 母亲孕前期及孕早期空气污染暴露与新生儿低出生体重患病风险有关联。

       

      Abstract:
      Objective To investigate the relationship of periconceptional air pollution exposure in mothers with the birth weight of neonates.
      Methods A three-year follow-up investigation was performed in the pregnant women who had regular antenatal examinations in a Three-A hospital of Nanning from January 2017 to January 2020. A face-to-face questionnaire survey was performed to collect related general information of the subjects, meanwhile, the data of the Air Quality Index (AQI) and the levels of PM2.5, PM10, SO2, NO2, and CO before pregnancy (12 weeks before pregnancy) and in early pregnancy (from the last menstruation to week 12 of pregnancy) were collected. A logistic regression model was used to analyze the influence of maternal periconceptional exposure to air pollution on the low birth weight of neonates.
      Results All 1326 periconceptional mothers included in this study gave birth successfully, and there were 42 low-birth-weight infants, accounting for 3.17%. The AQI, PM2.5, PM10, SO2, NO2 and CO in winter were higher than those in other seasons in Nanning. Multivariate analysis results showed that elevated AQI during pre-pregnancy and early pregnancy, pregnancy hypertension, placenta previa and placental abruption were risk factors of low birth weigh neonates, with OR (95%CI) being 1.619(1.114~2.355), 1.531(1.087~2.158), 1.603(1.151~2.233), 1.519(1.121~2.058) and 1.478(1.045~2.092), respectively.
      Conclusion Maternal exposure to air pollution during pre-pregnancy and early pregnancy is associated with the risk of low birth weight neonates.
